3 places ordered shut; Roaches spotted crawling on cornmeal and flour

Below is a list of places that were recently inspected by the Florida Department of Business and Professional Regulation.

According to state records, three places were ordered shut and a “follow-up inspection” was ordered at a third.

“Roach activity present as evidenced by live roaches found. 2 live roaches on back of hand sink in kitchen. Manager unable to kill as roaches went into wall behind seal of sink.”

“Dead roaches on premises. 1) Observed 1 dead roach on table behind tortilla chip cabinet in kitchen. 2) Observed 1 dead roach on floor at far end of bar in dining room. Employee removed both and cleaned both areas.”

“Interior of oven/microwave has accumulation of black substance/grease/food debris. Food debris in top of microwave cabinet.”

“Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling tiles in kitchen in front of cook line have heavy dust buildup. Repeat Violation.”

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately 35+ live flies flying in the dry storage area, approximately 8+ live flies touching dirty dishes by the dishwasher, approximately 10+ live flies on the wall by the handwashing sink in the kitchen and 3+ live flies in the dining room area. Observed 1 live fly flying into the reach in cooler when door opens. Repeat Violation.”

“Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Observed approximately 15+ dead flies next to the bug zapper at the dry storage room.”

“Wall soiled with accumulated grease, food debris, and/or dust. By the 3 compartment sink and handwashing sink.”

“Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or ware washing areas. Throughout the kitchen. Repeat Violation.”

“Objectionable odors in bathroom or other areas of the establishment. Observed a strange odor from the dry storage area.”

“No soap provided at hand wash sink.”

“Roach activity present as evidenced by live roaches found. Located at dry storage. Observed 5 live roaches crawling on a bag of corn meal and observed 2 live roaches crawling on a bag of flour.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Observed stains on cutting board with stains at preparation cooler at cook line.”

“Observed objectionable odors in walk-in cooler.”

“Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vent and tiles at ware washing area heavily soiled with dust. Observed ceiling vent above reach in freezer at cook line soiled with black substance.”

“Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed cook handling raw chicken and then changed gloves and began handling cooked chicken. Manager discussed proper hand washing procedures with employee.”

“Dollies, pallets, racks or skids used to store or transport packaged food not designed to be moved easily to allow for cleaning access. Observed food stored on crates inside walk in cooler and dry storage area not easily moveable for cleaning.”

“No proof of required state approved employee training provided for any employees.”

“Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed multiple ceiling tiles soiled. Repeat Violation.”

“Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed reach in cooler across cook line with ambient temperature of 50F. Observed no TCS food stored in reach in cooler. Observed reach in cooler next to kitchen back exit with ambient temperature of 50F. Observed no TCS food stored in reach in cooler. Repeat Violation.”
