Below is a list of places that were recently inspected by the Florida Department of Business and Professional Regulation.

According to state records, four were ordered shut and a “follow-up inspection” was ordered on another.

All the places mentioned were allowed to re-open following an ordered cleanup followed by a re-inspection.

“Rodent activity present as evidenced by rodent droppings found. Observed approximately 20+ rodent droppings on the 2 floor storage room behind the single service items, 15+ rodent droppings behind the AC unit on the 2nd floor.”

“Presence of insects, rodents, or other pests. Observed approximately 15+ centipedes dead throughout the storage area on the 2nd floor. Observed rodent.”

“Rodent rub marks present along walls/ceilings. Observed rodent rub marks on the floor under the single service items on the 2nd floor.”

“Dead roaches on premises. Observed approximately 6 dead roaches on the 2nd floor storage room. Observed 2 dead roaches in the kitchen behind the stairwell where containers of salt, rice and beans are stored.”

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately 7 flies in the kitchen by the prep area. Observed 1 live fly landing on the dough in the prep area.”

“Stop Sale issued due to food not being in a wholesome, sound condition. Observed 1 live fly landing on the dough in the prep area.”

“Dish machine not sanitizing properly. Discontinue use of dish machine for sanitizing and set up manual sanitization until dish machine is repaired and sanitizing properly. Observed Dishwasher’s (Chlorine 0ppm).”

“No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.”

“No proof of required state approved employee training provided for employee hired more than 60 days ago.”

“Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed at sushi counter prep table krab salad mix (55F - Cold Holding). As per cook for 1.5 hours. Chef placed into reach in cooler. Repeat Violation.”

“Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. See stop sale. Observed 3 rice cookers of sushi rice not time marked for time as a public health control procedures. As per chef sushi rice has been in place for 1.5 hours since opening.”

“Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed at front counter hot lamp tempura shrimp (77F - Hot Holding). As per cook for 1 hour, chef turned on lamp light.”

“Hand wash sink not accessible for employee use at all times. Observed at hand wash sink in kitchen across from dish machine blocked by serving trays and rolling cart. Observed at bar hand sink blocked by reach in freezer. Repeat Violation.”

“Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed cooked tomato (112F - Hot Holding) As per operator, tomatoes were on line under heat lamp less than 1 hour prior to inspection. Operator placed in steamer to keep warm.”

“Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw beef and lamb stored above raw fish in reach in at grill station.”

“Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed air vents in kitchen area soiled. Observed back of cooking equipment soiled. Repeat Violation.”
