Dirty Dining: Original Pancake House among 3 kitchens ordered shut last week

Roaches, flying insects found at numerous restaurants in South Florida last week

BROWARD COUNTY, Fla. – Records show The Original Pancake House in Coral Springs was ordered shut last week due to a roach problem.

All three state-ordered closures last week involve a flying insect problem.

Below is a list of places that were inspected and some of their violations.

All the places ordered shut were allowed to re-open following an ordered clean-up and then a re-inspection.

“Roach activity present as evidenced by live roaches found. Observed at ice machine in kitchen approximately 30 lives roaches. Additionally 2 roaches in oven at cook line.”

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed at storage room near expo line - approximately 10 small flying insects.”

“Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed at preparation area. Employee removed earbuds then put gloves on and continued to work with food without washing hands.”

“Water draining onto floor surface. Observed at hand washing sink in kitchen next to doorway.”

“Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Hollandaise Sauce was prepared at 8 a.m.”

“No soap provided at hand wash sink.”

“Roach activity present as evidenced by live roaches found. Observed at cook line under flip top cooler- 4 live roaches. Additionally 2 live roaches in oven at cook line.”

“Food stored on floor. Buckets of ginger stored directly on floor at front sushi bar and at storage room at back additionally Japanese seasoning stored on floor at storage room at back.”

“Raw animal food stored over or with unwashed produce. Observed at walk in cooler - salmon and tuna fish stored above avocados.”

“Time/temperature control for safety food held using time as a public health control not discarded at the end of the 4-hour/6-hour period. See stop sale. Observed at rear near reach in freezer. Pot of rice with 11:30 time label. Not discarded after 4 hours.”

“Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed at sushi bar grilled eel (89F - Hot Holding). Operator put in the oven to warm back up to 165F.”

“Stop Sale issued due to food not being in a wholesome, sound condition. Food contaminated by a live roach crawling on it.. See stop sale. Observed a live roach crawling inside the ice machine bin.”

Observed a live roach crawling in the ice machine. Observed a live roach in a picture frame. Observed three more live roaches coming out from between the floor and front counter.”

“Server handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Observed the manager take a customer drinking glass and put it in the hand sink and  went to the kitchen and continue to cook.”

“Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw chicken, beef and shell eggs over cooked black beans in the upright refrigerator.” Repeat Violation.

“No proof of required state-approved employee training provided for employee hired more than 60 days ago.”

“No soap provided at hand wash sink. Observed in both restrooms and front counter.”

“Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed at reach in drawer; ham (46F - Cold Holding); turkey (44F - Cold Holding) as per operator items not prepared or portioned today. As per operator items held out of temperature for 2 hours. Operator placed items in walk in cooler for quick chill. Repeat Violation.”
